NOTICE TO PATIENTS
No one will be denied access to services due to inability to pay. Light of Life Recovery & Wellness Services, LLC offers a discounted/sliding fee schedule based on family size and household income. Patients may apply for the Sliding Fee Discount Program at any time before or during treatment services. For additional information or assistance with the application process, please speak with our office staff.
2026 Sliding Fee Scale & Income Eligibility Guidelines
Eligibility for discounted services is based on household size and annual household income in accordance with the 2026 Federal Poverty Guidelines (FPG).

The following Federal Poverty Guidelines are used to determine eligibility for the Sliding Fee Discount Program (SFDP).
Sliding Fee Discount Program Policy
Light of Life Recovery & Wellness Services, LLC is committed to ensuring access to care for all individuals, regardless of financial circumstances. We offer a Sliding Fee Discount Program (SFDP) for eligible patients based on household income and family size in accordance with current Federal Poverty Guidelines.
Discounts may be available for uninsured, underinsured, and self-pay patients. Patients may apply for the program at any time before or during treatment services.

Sliding Fee Schedule
Light of Life Recovery & Wellness Services, LLC maintains a Sliding Fee Schedule (SFS) designed to provide reduced fees for eligible patients based on household income and family size. Discounts are determined using the current Federal Poverty Guidelines and may apply to qualifying behavioral health and wellness services.
The Sliding Fee Schedule includes discounted fee levels and nominal fees for individuals and families who meet eligibility requirements.
